ENTRY: Each team must return this entry form with
the entry fee of $40 by April 15,
2002. Additionally, each team
participant must return a properly filled out and signed Parent's Consent Form
with this Entry Form by that date.
HOUSING: A limited amount of housing may be
available. Housing will only be
available for participants accompanied by a designated Adult Team
Leader/Chaperone (who must stay with the competitor(s)). Availability is on a first come, first
served basis and arrangements must be made by each team individually. Requests
for housing must be made by April 15.
FOOD: An optional meal package is available;
please see below. Requests for meal
packages must be received with the entry form.
PAYMENTS: Please make one check for your entry fee,
meal packages and T-Shirts payable to The San Francisco Yacht Club, fax a copy
to the fax number below and return the original entry form to the address
below.
